When is wisdom tooth removal needed?
Repeated pain or swelling, food impaction, decay in the wisdom tooth or the tooth in front, cheek biting, cyst risk, or when orthodontist / surgeon has advised removal.
How is the procedure done?
After X-ray and local anaesthesia, the tooth is carefully removed. In surgical cases, a small flap and bone trimming may be done, followed by sutures and detailed post-op instructions.

When is wisdom tooth removal really needed?
Every wisdom tooth does not need surgery. But some conditions are risky and should not be ignored. This summary will help you understand when removal becomes important.

Common symptoms of wisdom tooth problems

  • Repeated pain or dull ache at the very back of the jaw.
  • Swelling of gums behind the last tooth or difficulty chewing on one side.
  • Food getting stuck in the gap or bad taste / smell from that area.
  • Pain while opening mouth wide, yawning or biting hard food.
  • Sharp pain radiating to ear / head, especially while chewing.
  • Decay in the tooth in front of the wisdom tooth on X-ray.
If you have 2–3 of these together again and again, surgery is usually advised after seeing the X-ray.

Types of wisdom tooth position & cost idea

  • Vertical / straight: Often simple removal if fully erupted.
  • Tilted (mesio / disto-angular): Food trapping, decay risk – may need surgical removal.
  • Horizontal / deep impacted: Usually surgical with flap, bone trimming & sutures.
  • Partially erupted: Repeated infection around gum flap (pericoronitis).

Swelling and mild–moderate discomfort – usually controlled with medicines & cold compress.
Days 4–7
Improvement
Swelling reduces, normal daily activities become easier. Soft diet still advised on that side.
After 1 week
Review / stitches
Review visit for suture removal or check-up. Most patients are comfortable by this time.
Before surgery – how to prepare
  • Eat a light meal 1–2 hours before the appointment (unless advised otherwise).
  • Inform your doctor about blood thinners, diabetes or any regular medicines.
  • Arrange someone to accompany you, if you feel anxious about dental treatment.
  • Avoid smoking / alcohol before the procedure.
After surgery – important instructions
  • Do not spit or rinse forcefully for the first 24 hours.
  • Use prescribed medicines exactly as advised.
  • Apply cold packs on the outside of the face for the first day (as instructed).
  • Stick to soft, cool food; avoid hard, spicy or very hot items initially.
  • Call the clinic if pain/swelling suddenly worsens after a few days.
